 Role and Responsibilities

The Community Health Nurse [CHN] provides health services that promote the prevention of disease, maintenance of health and the overall health and well-being of individuals, families and the community.

The Community Health Nurse initiates primary care treatment including health care management and therapeutic interventions in accordance with nursing statutory and regulatory standards, limits, and conditions, and employer policies and procedures. 

The CHN collaborates with patients/clients and other health professionals to identify and assess trends and patterns that have implications for patients/clients; develops and implements population and evidence-based strategies to improve health and healthy living for patients/clients. 

Delivers patient/client care in accordance with established standards, policies, procedures and the goals, objectives, guidelines and directions of Health Manager by performing duties such as:

  • Performing nursing skills, such as delegated medical functions and crisis intervention, such as emergency assessment and care including arranging for escort and evacuation for further intervention as required, point of care testing, specimen acquisition- including venipuncture, and arranging referral where necessary.
  • Assessing, planning, implementing, documenting and evaluating the care needs and goals for individual patients/clients and their caregivers/families.
  • Processing and implementing physicians and or Nurse Practitioner orders, identifying and seeking clarification as necessary.
  • Providing information to staff, including patient/client specific care instruction.
  • Uses evidence and research and synthesizes nursing and health knowledge from a broad range of theories, models and frameworks to address the health status of the population as well as the factors that influence the delivery and use of nursing and health services.
  • Provides health education and promotion and treatment services for patients/clients, including individuals, families, groups and communities.

 What You Will Bring to The Role:  

  • Baccalaureate Degree in Nursing and/or Registered Nurse.
  • Current practicing registration with the BC College of Nurses and Midwives.
  • Basic Life Support, Level “C” certificate.
  • Acceptable experience in a clinical health setting or cross-cultural health promotion field.
  • Transportation of Dangerous Goods (TDG) course.
